Our local facilities provide face-to-face consultations for routine checkups, chronic condition management, and immediate care. Additionally, our memberships include virtual access to primary care, allowing you to consult with providers online as an added convenience.

*Note: These services are available with visit fees and no limit on number of visits per year.

The visit fees per service are: Office Primary Care Visit - $20; Specialist Care Visit - $50; Urgent Care Visit - $50.
Lab work is reimbursed up to $250 per year.
